Can you instruct me how to take a document that opens in Adobe
Reader...It is like a form to fill out with squares to write in, etc...
How would you make that so that it could be opened and the blanks

You'd either need the full version of Adobe, which is very expensive, or there are some utilities which will convert PDF to other file types.
I did a search on Google for "pdf to doc" and got a lot of listings - I then refined the search using "freeware" to get free utilities. I haven't tried any myself, so I can't recommend one.
